Medical Expense Shield in North & South Carolina
Medical Expense Shield pays cash to you from day one, on the Aetna Open Choice PPO network with Mayo Clinic in-network.

Cash Paid From Day One
Most people have two complaints about health insurance. First, the premium is expensive. Second, they still owe thousands out of pocket before the plan pays anything. Medical Expense Shield from Allstate Health solves the second problem.
It bundles short term medical with a fixed-benefit plan, so cash is paid directly to you starting with your very first doctor visit, ER trip, or hospital stay, before the deductible is ever met. Traditional health insurance makes you pay a deductible first, often $5,000 to $10,000, before the plan covers anything meaningful. This plan flips that. The fixed-benefit component pays you cash from your first covered event, and that cash counts toward the deductible at the same time.
What You Get
- First-dollar cash: Benefits paid to you from day one of covered care.
- Counts twice: Every dollar of cash benefit also works toward meeting your deductible.
- After the deductible: The plan pays 100% of most covered care, and the cash benefits keep coming.
- Your choice of terms: Multiple deductible tiers, Core or Plus benefit levels, and rate-lock options.
- Network: Nationwide Aetna Open Choice® PPO, with Mayo Clinic in-network.
A Network That Follows You
The Aetna Open Choice PPO gives you national access, including major providers throughout North Carolina and South Carolina. You are not restricted to a narrow county-based network the way many Marketplace HMOs are. If you split time between the Carolinas and somewhere else, or you travel often, this network goes with you.
Who Medical Expense Shield Fits Best
Medical Expense Shield is not the right fit for everyone, and I will be direct about that. It is typically the best fit for:
- Self-employed people and small business owners who do not get a big Marketplace subsidy
- Early retirees bridging the gap until Medicare
- Healthy families who want lower monthly premiums than ACA plans typically cost
- Anyone whose current plan has a deductible so high they cannot imagine using it
Because the short term component is medically underwritten, it is a poor fit if you are managing an ongoing chronic condition. In that case, an ACA Marketplace plan with guaranteed-issue protection is almost always the better answer.
Medical Expense Shield vs. ACA
| Comparison | The short answer | The trade-off |
|---|---|---|
| Medical Expense Shield vs. unsubsidized ACA | Typically much cheaper for healthy people who do not qualify for a big subsidy. | Medical underwriting applies. |
| Medical Expense Shield vs. subsidized ACA | If you qualify for a substantial subsidy, the ACA plan is often the better math. | I run your actual numbers. |
| Either option vs. going without coverage | Not a comparison. | Do not do this. |
